Portugal to Overhaul Pay for Locum Doctors in Emergency Shifts

Portugal’s public health system is set to introduce new pay rules for locum doctors working emergency shifts. The Cabinet will soon approve a decree outlining bonus payments for extra hours. These changes aim to regulate incentives for physicians covering additional shifts in hospitals.

The bonus system applies to full-time doctors who complete 250 supplementary hours and others who work 150 extra hours. Payment is triggered once the additional hours are finished or if the doctor is not needed for the full shift due to service constraints—provided their availability was confirmed by the institution. The bonus does not count as part of base pay and will not affect other supplementary earnings.

The incentive is calculated in 48-hour blocks, with the percentage rising by 20% if a doctor works at least 48 hours on weekends and agrees to an extra shift. Before these blocks are counted, physicians must first use up the supplementary hours already allowed by law. Monitoring of work beyond annual legal limits for extra hours will be handled by the clinical director and the head of the emergency department. The exact bonus amounts remain under discussion with medical unions. Unlike standard pay adjustments, this decree has no fixed end date, meaning the rules will stay in place until further notice.

The new decree will provide structured pay for locum doctors taking on emergency shifts. Oversight will ensure compliance with annual hour limits, while negotiations continue on final bonus figures. The system is designed to reward extra work without altering base salaries or other supplementary payments.
