Crinetics' PALSONIFY shows strong long-term results in acromegaly trials

Crinetics Pharmaceuticals has released new long-term data for its acromegaly treatment, PALSONIFY (paltusotine). The findings were shared at ENDO 2026 during an oral presentation on the drug’s efficacy and safety. The study results cover multiple trials, including PATHFNDR-1, PATHFNDR-2, and ACROBAT Advance. In PATHFNDR-1, patients on PALSONIFY maintained stable IGF-1 levels at 0.81x the Upper Limit of Normal (ULN) after 96 weeks. Acromegaly symptoms and pituitary tumour volumes also showed no significant changes during the study.

PATHFNDR-2 reported a drop in IGF-1 levels from 1.64x ULN at the start to 0.96x ULN after 72 weeks. Additionally, 7 of the 83 participants saw a reduction of over 20% in pituitary tumour volume within 24 weeks.

The ACROBAT Advance study found that combining PALSONIFY with oral cabergoline improved IGF-1 control. The treatment was well tolerated across all trials. Safety data from a pooled group of 167 patients in the open-label extension (OLE) showed no new concerns, with the most frequent side effects being diarrhoea, joint pain, headaches, and urinary tract infections.

After two years, PALSONIFY proved effective for patients who switched from monthly injectable somatostatin receptor ligands (SRLs) and for those previously untreated. The results confirm PALSONIFY’s long-term effectiveness and safety in managing acromegaly. Stable IGF-1 levels, tumour volume reductions, and a favourable tolerance profile were observed. These findings support its potential as a reliable oral treatment option for patients.
