It should also specify the prescribed dose, timing of administration, and the medical reason for use. Patients are advised to carry this information during doctor's visits or hospital stays so that healthcare professionals can make informed decisions.

Primary care physicians are responsible for creating and updating the plan, either on paper or digitally. The electronic version, introduced in 2022 under § 388 SGB V, has grown rapidly in adoption. By early 2026, around 40% of pharmacy prescriptions included an eMP, with higher rates in states like Bavaria, Baden-Württemberg, and North Rhine-Westphalia. Major health insurers such as AOK, DAK, and TK reported usage exceeding 50%.

These plans are particularly valuable for patients with multiple conditions or those in long-term care. They allow doctors, nurses, pharmacists, and caregivers to track medications closely and avoid harmful interactions.

The push for medication plans reflects a broader effort to improve patient safety and streamline care. With electronic versions now widely used, doctors and pharmacists can access up-to-date records more easily. Patients are encouraged to keep their plans current and carry them during medical appointments.
