Common asthma drug may reverse liver damage in MASH patients

Scientists at the Medical University of South Carolina (MUSC) have uncovered a promising new approach to treating metabolic dysfunction-associated steatohepatitis (MASH). Their research suggests that formoterol, a drug traditionally used for respiratory conditions, could be repurposed to reverse liver damage in patients. Early findings indicate it may offer advantages over existing treatments, which often fall short in restoring liver function. Formoterol belongs to a class of drugs known as beta-2 agonists. Retrospective patient data has shown that these medications may improve liver-related outcomes. In animal studies, formoterol successfully reversed fatty liver pathology linked to MASH by promoting mitochondrial biogenesis—helping liver cells regain metabolic balance.

Unlike the two currently approved MASH treatments, resmetirom and semaglutide, formoterol appears capable of actively repairing liver damage rather than just managing symptoms. These existing drugs also come with side effects and limited ability to undo harm caused by the disease. A clinical trial is now underway to explore formoterol’s potential in diabetic kidney disease, which shares underlying mechanisms with MASH. However, further testing is required to confirm its effectiveness, establish the right dosage, and assess long-term benefits for patients.

If proven effective, formoterol could become a cost-effective and relatively safe option for MASH and related conditions. The drug’s repurposing may provide a much-needed alternative to treatments with limited reversibility and unwanted side effects. Researchers will continue evaluating its potential in ongoing and future clinical studies.
